Dear Mercer County Employer –
The Community Health Improvement Plan (CHIP) initiated by Mercer Health, Celina-Mercer County Department of Health and Gateway Outreach Center has identified three major health concerns in Mercer County. The CHIP committee is working to increase awareness and improve these areas which include; alcohol use and abuse, drug use and abuse, and obesity. As a local employer, we need your assistance.
As indicated, our community has identified obesity (lack of physical activity and unhealthy lifestyle choices) as a major health risk impacting the health of our residents. These individuals also comprise the workforce for our local businesses and often impact their productivity and profitability. Because employees spend the majority of their waking hours at their place of work, the environment of the company has a large impact on their healthy lifestyle choices (smoking, physical activity, healthy food choices, etc.).
2014 marks the initial year of our Community Health Improvement Plan (CHIP) to address obesity in our community, and we need to collect information about what wellness resources local employers currently have available. Even if you don’t currently have a Wellness Program, we need your input to develop our baseline data, and help determine our focus for the future and how we can best help our local businesses.
As a local employer and business leader, we need you to complete the attached survey, which should only take a few minutes of your time. If you don’t currently have a Wellness Program, there are only 6 questions you need to answer. If you do have a Wellness Program, we have several more questions that will provide us with additional information about what is included in your program and how your worksite is able to provide these services. We are asking that all surveys be completed by September 5, 2014.
Take the survey here: https://www.surveymonkey.com/s/CHIPWorksiteWellness
We thank you in advance for completing our survey. We ask that you share this email/letter with other local employers by forwarding this email or sharing the link on Mercer Health’s Facebook page.
If you have any questions about the survey, please contact Renee Kinney, Director of Wellness and Occupational Health Services at Mercer Health at 419-586-1220.
